Industry experts in Healthcare & Aging Services.

Most expense management firms serve every industry and specialize in none. Decision makers in Acute Care, Post-Acute Care, PACE, and Seniors Housing and Services rely on us because our subject matter expertise and 17 years of industry experience.

ACUTE CARE

Margin that hospitals and health systems leave on the table

Hospital margins remain under pressure as non-labor costs outpace reimbursement growth. Purchased services and supply chain now account for 20% to 45% of total hospital expense, and both are projected to keep climbing through 2026, per Vizient's Winter 2026 Spend Management Outlook and research from Valify. Categories like food services, environmental engineering, revenue cycle support, and IT and equipment maintenance are usually reviewed once, at signing, and rarely re-benchmarked against what comparable systems actually pay. That gap is where the greatest margin recovery lives.

POST- ACUTE CARE

The savings operators can’t see from the inside

Skilled nursing, rehabilitation, and long-term care operators are managing some of the thinnest margins in healthcare, squeezed between a shifting reimbursement mix and rising costs across the board. Plante Moran's 2026 Skilled Nursing Facility Benchmarking Report, drawn from more than 12,000 facilities nationwide, confirms these pressures are showing up sector-wide. External benchmarking is the tool CFOs rely on to find overspending, but it only works with a large, comparable peer set, something few operators can assemble on their own. Dining services, pharmacy, rehab, facilities management, and medical supply procurement are where post-acute providers most often overpay.

PACE

Protecting the capitation dollar in PACE

Because PACE organizations operate under a fixed, per-member-per-month capitation rate, program revenue doesn't move when costs do. Every dollar saved on the expense side goes directly to program sustainability and margin. Research from Health Dimensions Group's PACE Technical Assistance Center identifies provider services, durable medical equipment, transportation, dialysis, and institutional care as the largest, most variable cost categories and the greatest financial risk to program success. Because most PACE organizations serve a single county or region, they rarely have visibility into what comparable programs elsewhere pay for the same services.

SENIORS HOUSING & SERVICES

Finding the savings your community can’t benchmark alone

Labor remains the largest line item in senior living operations, accounting for roughly 55% of total operating expense industry-wide according to NIC MAP Vision data, and it's rightly where most operational attention goes. But ongoing LeadingAge and Ziegler research into Life Plan Communities points to added pressure from rising non-labor costs: dining, pharmacy, rehab, facilities management, and medical product procurement. A single nonprofit community, or even a multi-site system, rarely has visibility into what comparable operators in its region actually pay for the same services.

WHY SPECIALIZATION MATTERS

11_accent_hairline_light.png

Most firms benchmark against averages.
Expense Consulting  benchmarks against your peers.

Typical firms can tell you what an office park pays for waste hauling. It cannot tell you what a 240-bed skilled nursing facility should pay for regulated medical waste disposal, what a Life Plan Community should pay for dining management, or how a PACE organizations should structure their third-party services contracts.

Expense Consulting's experts specialize in the areas of spending they support. When we engage with your operational teams and vendor partners we bring our understanding of industry best practices and pressure points.
